Posted 2025-06-20Company : EndorsedJob Description :JOB SUMMARYJob Description SummaryRemote/Hybrid - Must be in Pittsburgh and surrounding areas.Enhanced Community Care Management (ECCM) is currently seeking a social worker to expand its growing care delivery team. ECCM is the region's premiere Palliative Care Service and meets with patients in the home and virtually, providing supportive care services. The incumbent will assess patients for psychosocial complexity and provide targeted interventions based on patient goals. Executing as a community-based treatment team, ECCM works in conjunction with PCP’s, specialists, hospitals, and post-acute care facilities to meet the social, psychological, and medical needs of patients who have complex conditions or terminal diagnoses.This position is an integral part of the care delivery system working together as part of an interdisciplinary team to provide community based palliative care. The incumbent will provide palliative care to complex patients across our footprint. The focus on advance care planning, what matters most, and development of goal concordant care plans. The incumbent will work within their scope and standards of practice and within the policies, values, mission of the organization, placing the customer first.Monday-Friday. No evenings. No weekends. Pittsburgh and surrounding area.ESSENTIAL RESPONSIBILITIES
- Completes and documents psychosocial assessments, intervention, and coordination of care to identified populations.
- Provides crisis intervention/counseling to identified populations to enhance patient/family coping mechanisms regarding illness.
- Engages the patient/family/significant other and interdisciplinary team in the discharge process to identify appropriate post-acute resources and community service follow-ups.
- Provides education to patients/family/significant other and interdisciplinary team regarding psychosocial factors related to illness.
- Actively participates in those areas of expertise to further enhance the role of social work within the hospital.

- Master's Degree in Social Work
- 6 years of related and progressive experience in lieu of Bachelor's degree
- None
- 5 years of Post-graduate experience with experience as a Social Worker in an inpatient, outpatient, home health, or other healthcare setting where medical social work services are provided
- 2 years Case Management related activities
- None
- Licensed Social Worker (LSW) OR Licensed Clinical Social Worker (LCSW)
- None
- Understanding of the importance of cultural competency in addressing targeted populations
- Understanding of the role of lifestyle in disease processes
- Basic computer skills, including Microsoft Office products
- Ability to work collaboratively and function as a team member in a professional environment
- Demonstrated ability to manage multiple priorities and perform work independently
- Evidence of ability to analyze outcomes and develop goal-oriented action plans
- Demonstrated analytical and decision-making skills
- Motivational interviewing competency
